End of Year Recital Registration Form

It is expected that school age children will participate in both performances (1.00pm and 5.00pm). Failure to be present for both performances can cause disruption and disappointment to other children.

If you know on registration that your child can only participate in one performance, please contact the office, or speak to your class teacher to make sure your child's absence will not affect the rest of the group.

My child can only dance in one performance, I acknowledge that I need to contact the office directly to make arrangements, as I am aware dancing in only one performance may not be possible.
